Classification:

  • Antipsychotic

Mechanism of Action:

  • Blocks post synaptic dopamine receptors in the brain

Indications:

  • Treatment/management of acute/chronic psychosis, aggressive or agitated behavior

Contraindications:

  • Parkinson’s
  • CNS depression, coma
  • Underlying correctable causes
  • Previous history Neuroleptic Malignant Syndrome

Precautions:

  • May impair mental and physical abilities
  • Use with caution in patients taking anticoagulants
  • Diphenhydramine should be available

Adverse Effects:

  • Extra pyramidal reactions treat with Benadryl 25 – 50 mg IV/IM
  • Lowers seizure threshold
  • Blurred Vision
  • Dry mouth
  • Urinary retention
  • Hypotension
  • Tachycardia
  • Precipitation of angina
  • Rarely: neuroleptic malignant syndrome
    • High fever, muscle rigidity
    • Possible confusion, muteness
    • May see profuse sweating, drooling
    • Risk of cardiac and or respiratory arrest, seizures, renal failure
  • Prolonged QT interval

Dose:

  • 2 – 5 mg IV/IM PRN
