Classification:

- Non-depolarizing neuromuscular blocking agent

Mechanism of Action:
|
- Prevents acetylcholine from binding to receptors at the muscle and end plate, blocking depolarization
|
|
Indications:
|
- Maintains paralysis post intubation after RSI
- Maintain patient and crew safety in a combative patient, after patient intubation
- To ease ventilation and/or oxygenation of intubated patients
|
|
Contraindications:
|
- Hypersensitivity to bromides
- Unable to ventilate the patient with BVM
|
|
Precautions:
|
- Use cautiously in underlying neuromuscular disease, due to risk of prolong effects
- Use cautiously in hepatic disease
- Use cautiously in obesity
- Use cautiously in bronchogenic cancer
- Use cautiously in electrolyte disturbances
- Emergency airway management equipment, and those skilled in its use, must be readily available at all times during administration.

Adverse Effects:
|
- Bronchospasm, hypotension, tachycardia, dysrhythmias, N/V
|
|
Dose:
|
|
|
Adult:
|
- 0.6 - 1.2 mg/kg IV, repeat q 30 min at 0.1 - 0.2 mg/kg prn
|
|
Pediatric:
|
- 0.6 - 1.2mg/kg IV, repeat q 30 min at 0.1 - 0.2 mg/kg prn effect may prolong with peds
